Caprani Care aims to offer a supported living service to adults with learning disabilities, mental health conditions, physical health conditions, and/or physical disabilities and behaviours that challenge in a range of settings.

We aim to support our service users who display behaviours that challenge by implementing positive behaviour support. We aim to identify and understand the reasons for their behaviours, producing strategies for implementation. The service specifically aims to deliver a high standard of care and support to those who choose to live in their own home. The individuals who choose our service are likely to require physical assistance, prompting and/or supervision on a daily basis, with activities such as dressing, bathing, eating and drinking, oral care and care of skin, hair and nails, all of which are described by the regulated activity known as personal care.

We also provide round-the-clock support in the form of live-in care. This enables individuals to continue living in their own familiar surroundings with their loved ones. This mitigates such a significant transition from their own home to an unfamiliar environment or care facility, which can be extremely anxiety-provoking and unnecessary for some individuals. We support individuals to maintain their known routine and offer support with personal care, diet and nutrition, activities, accessing the local community, medication, mental and physical health, etc. We provide 1:1 personalised support that is flexible and adapted to meet the preferences for the individuals. We recognise the importance of providing support and relief to family members that reside within the same home and ensure that this is in place, in a cost-effective manner when compared to nursing and care homes.

We aim to promote our service users’ independence by encouraging them to develop new skills using a range of staff support, technology and resources. We aim to develop their independent living skills such as cleaning, budgeting, shopping and safely managing their medication. Our service also aims to observe and monitor the mental health of its service users and communicate any concerns with the relevant professionals in a responsive manner.

We aim to support our service users with growing older by aiding their understanding of what this could mean for them physically and mentally. We aim to support them to prepare by encouraging end of life care planning and advance care planning. This is to ensure that their wishes and beliefs are taken into consideration prior to crisis point.

We also aim to support service to use our outreach services and access amenities within their locality in a safe way. This includes visiting libraries and day services, ensuring that they remain a part of their community.

As a service we aim to ensure that we have provisions in place to responsively and effectively manage a crisis whether it be locally or globally, putting service users’ safety and wellbeing at the forefront.

We aim to encourage and support all types of relationships that our service users choose. Ensuring their safety is maintained and overall improving their quality of life and wellbeing.

Caprani Care’s objectives are:
– To provide a high standard of personal care for those who use the service.

– To provide a thorough assessment of each individual’s need for personal care.

– To ensure that the care received is continuously assessed and evaluated responding to any changes. That the views and experiences of the service user and their families are valued, acted upon and that the care is tailored to them in a personalised manner.

– To provide individual, personalised support plans for each service user that are easily accessible to them. These support plans will identify each need in relation to their care and include a clear plan of how their needs will be met in a safe, effective, caring and responsive manner. Ensuring that choice, inclusion and independence is promoted.

– To provide personal care by fully trained and DBS-checked support workers. The care must be delivered in a kind and respectful manner, protecting the service users’ dignity. It will be carried out by those of good character and health.

– To provide choice with regard to the location where the care in received.

– To actively promote equality, diversity, privacy and dignity in the provision of personal care.

– To ensure that the service user understands the support that they are receiving, using a range of resources.

– To obtain consent to the personal care (where the service user is able).

– To ensure that the nutritional needs of the service user are being met, including the need for choice and respecting any personal or religious preferences.

-To ensure that arrangements are made to safeguard the service user from abuse in relation to all aspects of the support and care they receive.

– To co-operate with other providers involved in supporting or caring for the service user.

– To work closely with the families and carers of the service users.

At ACE Day Service we focus on skill development and social interaction. We endevour to offer a range of activities that meets preferences and facilitates positive outcomes to be achieved. We aim to offer a personalised activity program that is dynamic and adaptive to suit the needs of all attendees. We acknowledge that every individual has different strengths, abilities and choices. We aim to identify social connections and provide a service that enables the development of these. Each service user will have an activity preference and engagement level assessment, and this will be updated regularly. Exciting, fun and stimulating activities will be available and refreshed regularly to maintain interest and skill development.

– We endeavour to ensure that personalised support needs are met whilst service users attend the day service. Our priority will be to improve the well-being, quality of life and opportunities for all service users that attend.
